For the institution/company where the internship will take place to be deemed eligible, the FR-1392 Internship Site Information Form must include:
- In the “Company Business Information” section, at least one checkbox other than the “Other” option must be checked.
- In the “Company Engineers and Number” section, there must be at least one engineer listed, excluding the “Other” option. If the engineers at the company can only be listed under the “Other” option, this situation must be discussed with the workshop internship committee.
